Our Process
Referral: After the submission the referral form, we will get in touch with you to ask some
questions and arrange an intake and / or information meeting.
Intake: We will discuss in great detail about the program and address any of your inquiries. At
this meeting, all necessary paperwork is finished upon approval, which completes this stage of
the process.
Discovery: Our trainee will meet with the individual to discuss about their requirements and
preferences. The completion of corresponding assessments enables our team to provide the
individual with better service.
Support & Training: From job searching to skills training, we will help you along the pathway to
achieve the objective discussed at the intake meeting.
On-going Support: We will always be ready to support your program& goals either be to secure
job and maintain employment or develop the skills to live independently. At team meetings,
outcomes are discussed and progress is evaluated. Your objectives could be re-viewed and
new goals may be set to support desired results.
Brighter Home allows us to help you with care you need both in primary support and more
intrusive support services. Minnesota Statutes, Chapter 245D, controls the delivery of services
to people with disabilities and those 65 and older that are based in the community and at home.
245D Services can assist you in obtaining or regaining independence in your home and in the
community. Our highly qualified caregiver will assist you in achieving your goals and objectives
by first learning about what is important in your life and then devising a strategy to get you there.
Our team keeps you connected to your community and assists you in completing significant
