Services

A new housing initiative called Housing Stabilization Services (HSS) is financed with federal Medicaid (Medical Assistance) funds. We provide the Housing Access Services Program (HAS) continuously with the updated version.   Regardless of the program, we consult with each client individually to determine what they want and require in terms of housing. T this might vary substantially from person to person.

HSS
Housing Stabilization Services (HSS) is a new Home and Community based Service (HCBS)
that supports people with disabilities in finding housing; setting a basic living budget; creating
natural community supports by connecting with landlords and neighbors; and assisting people
with understanding the rules and responsibilities of a lease.


Goals of HSS are to:
• Assist people in finding housing
• Boost the community in long-term stability in housing
• Prevent recurrent homelessness or institutionalization in near future

We provide housing transition and also housing sustainment. Those in need of assistance
finding, locating, and moving into housing are provided with transition services. Those who
require assistance to continue living in their existing dwelling are given sustaining services.
To qualify for HSS, you must meet the following criteria:
• 18 years of age and receiving Medical Assistance
• have a handic or a chronic condition
• Be experiencing instable housing
• have a requirement for service due to restriction brought on by a personal disability
Note: Admission into Placement Partners HSS program is subject to State of MN Department of
Human Services (DHS) Eligibility Verification Review.
Documents needed for DHS Eligibility Verification Review:
• Professional statement of need, OR
• Medical opinion form, OR
• Proof of recipient of SSI or SSDI
ALSO
• MN Choices Assessment, OR
• Long term Care Coordination Plan, OR
• Coordinated Entry Assessment
HSS program will be available starting on July 20th, 2020.
Housing Access Coordination program participants may transit into the new HSS program at the
time of waiver renewal.

Basic

24-Hour emergency assistance- Our staffs provide on-call counseling and problem-solving, as
well as quick support at a persons house in the event of a medical or personal emergency.
Adult companion services – Our companion services offer emotional support and non-medical
monitoring to improve socializing experiences and improve your life.
Homemaker services- When you are unable to do so, our experienced Homemakers may assist
you with general cleaning of your house.
Night supervision- Offers nighttime support and surveillance inside the persons home by awake
personnel.

Personal support- Services are offered in a persons home or community to assist them in
reaching their full potential, improving their independence, and attaining their community
participation goals.
Respite care services- We provide short-term care services to guarantee your health &
wellbeing within absence of your legal guardian or family member(s).
Individual community living supports (ILCS)- For those who require recalls, cues,
intermittent/moderate monitoring, or physical help in order to stay in their own homes.


Intensive support services

Semi-Independent living skills – Services required for a successful community life by an adult
with a developmental impairment or associated condition(s).
In-home family support- Services are provided to a person including hisher extended family
members at their house or in the community in order for the individual to remain or return to
their residence. This includes teaching the individual and family members to improve their ability
to care for and maintain the individual at home.
Supported living services- A person who requires daily staff help for problematic behaviors,
medical requirements, physical needs, and/or life skills. Supported living services is offered to
children under the age of 18 who reside outside of their birth or adoptive home.
Independent living skills training - Services that help people acquire, retain, and enhance their
community-living abilities.

Employment services

Employment development services - Personalized assistance to assist a person in obtaining
competitive, integrated employment, becoming self-employed, or establishing a microenterprise
firm in his or her neighborhood.
Employment exploration- Services that assist individuals in gaining a better awareness of
competitive, integrated job possibilities in their neighborhood. Exploration activities and
experiences build a persons knowledge, interests, and preferences, allowing him or her to
make educated judgments regarding competitive employment.
Employment support services: Personalized services and assistance that help people keep their
jobs in public institutions or enterprises. Employment assistance is provided in integrated
community settings.
